Job details:  Intermediate Care Team. Immediate start.

• Completion of initial assessments for patients newly admitted to Intermediate Care.
• Setting therapy goals and use of Criteria Led Discharge.
• Completing therapy reviews following patients fracture clinic appointments.
• Completion of access visits as required.
• Assessment of and provision of equipment.
• Supporting Band 4 Therapy Assistant Practitioners and Band 2 and 3 Integrated Care Workers.
• Attendance at and feeding back in MDT meetings. 
• Discharge planning. 
• Attendance at professionals’ meetings and case conferences.
• An enhanced DBS will be required for adults.

Working hours: 37.5 hours per week. Monday to Friday, however, can be flexible.
