Job details:  To start asap. To cover mat leave and current vacancy. Older adults MH. Community.
Working with patients in their own homes/nursing homes. Experience with crisis teams or routine CMHTs Complete initial assessments, risk assessments and crisis plans. Triage referrals into the team, duty clinician.
Mixed caseload of patients with a functional and organic MH condition.  CRHTT incorporates dementia intensive support team. Working hours: up to 37.5 hours per week. 7 days a week 8am to 8pm.
