Job details:  Physical Adults – Complex Discharge Planning – reablement and Manual Handling. 3 months. Access to own car required. Previous Ward experience is Ideal, previous working in a MDT preferred, good caseload management skills. Good IT Skills are required for electronic record keeping. Cross-site cover preferred. Shifts are available across 7 days, hours of work are core service hours (0900-1700, 0830-1630 or 0800-16.00). shifts are 7.5 hrs but open to staff who can only work shorter shifts to accommodate school pick-ups etc.
