Job details:  Out of Hospital Care – Surgery Therapy Services. Integrated Discharge Team. JD available on request: The IDT Occupational Therapy service is provided across in –patient medical wards, working in partnership with a wide variety of patients, professionals and carers to support informed decision making in relation to safe, effective discharge with a focus on function and meaningful activity.
The team works with patients for rapid assessment and discharge as well as with individuals with more complex needs within the hospital. This may include fast track discharges, end of life care or those who’ve had a longer admission due to multifaceted presentations including complex physical, cognitive, and mental health needs, as well as complex social situations and environments. 
The clinical work is largely in the acute setting at the Conquest but may also require visits to clients’ homes, covering an area which includes Rye, Bexhill and Robertsbridge. Car driver essential. Preferable to have own car but pool car may be available upon request (dependent on demand and availability). Working hours: shifts, 08.30 – 16.30.
