Job details:  To start asap. Community services. Pathway 2/3 Intermediate Care. Care of the older person Rehabilitation in a non-acute inpatient setting. Discharge planning skills. MDT working Clinical decision making. Will work as part of an established inpatient intermediate care team alongside PT, RA, pharmacy, nursing team, discharge facilitators & ASC. Note that there is free onsite parking. Working hours: shifts, 09.00 – 17.00. Full time preferred, could consider part time.
