Job details:  Physiotherapist/Occupational Therapist/Adult Speech and Language Therapist required. Seeing patients with a Sutton GP, both in their own home, care homes & in the community as required. To holistically assess review & support patients referred & accepted to the service in a timely manner in order that contractual obligations are met & those patients’ needs are effectively identified & appropriate interventions put in place.  In line with local & national standards & guidelines. Car driver essential. JD available on request. Working hours: 5 days a week Mon- Fri; excluding bank holidays. 7 hours a day with 30 min unpaid lunch. 35 hours a week, typically 08.30-16.00.
