Job Title:  Assistant Practitioner – Band 4

Job details:  To start asap to cover sickness. Assistant Practitioner required with Therapy and community background, some NHS experience, UK driving license and experience of working with older adults. Car driver required as it will be based in the community. JD available on request:

• To assess service users needs within an agreed framework and plan, deliver and evaluate interventions under the supervision of a qualified practitioner.
• To carry our assigned duties, involving direct care and observation of service users under guidance and supervision of a qualified practitioner.
• To report to the qualified practitioner and observations made whilst providing care to serviceusers.
• To provide supervision and guidance to assistant support worker and support workers.

Working hours: Full time. Monday to Friday, 08.00 – 16.00.
