Job details:  To cover vacancy. The @Home Service are a Multidisciplinary team (MDT) of Health Care Professionals offering both a Comprehensive Geriatric Assessment (CGA) and the facilitation and administration of IV antibiotic therapy as an alternative to hospital admission whilst supporting earlier discharges to those patients receiving IV antibiotic therapy. The patient cohort are over 18 years of age in the RCT, Merthyr and Cynon localities. The team are based at Dewi Saint Health Park where both clinic sessions and domiciliary visits are offered to assess, plan care and educate to avoid future hospital admission. The MDT team includes Advanced Nurse Practitioners, Registered General nurses, Nurse practitioner in Mental Health, Health Care Support Workers, Administrative colleagues, Speech and Language Therapist, Occupational Therapists and Physiotherapists. Through a medical model supported by the in-house Associate Specialist and medical colleagues the team offer a 5 day a week assessment alongside therapy colleagues whilst during out of hours & weekends the service is nurse led. The criteria for referral is: Frail elderly patient approaching crisis Challenging Polypharmacy with complex physical comorbidity Frequent or recurrent unexplained falls Unexplained weight loss, unlikely to be malignancy Acute gait & balance problems, sudden deterioration in mobility For consideration of IV antibiotics at home. The OT will undertake home based assessments at pace, often responding same day with a view to preventative and admission avoidance interventions being delivered including delivery and fitting of equipment. Working hours: Wednesday, Thursday, Friday, 08.00 – 16.00.
