JD available on request:  The post holder will work as a specialist autonomous practitioner to perform advanced assessment of patients with diverse presentations. To use advanced clinical reasoning to diagnose/interpret and then develop, deliver, and adapt individualised treatment plans for patients with communication and swallowing deficits. This includes provision into video fluoroscopy clinics and critical care, as well as patients with a tracheostomy. Working hours: 0.7 WTE. Weekdays with the option to participate in inpatient stroke rota.

Job Details:  The new sub-acute wards are additional community step down inpatient ward setting based on site at Shrewsbury Hospital. A registered Physiotherapist is required to assess and treat patients requiring ward-based rehabilitation and assessment for discharge planning. Experience of working independently and at pace is required to ensure proactive discharge planning and robust and effective treatment plans are in place.
The person will be part of a therapy team including a rehab technician and an occupational therapist. Experience of community-based pathways would also be desirable to ensure length of stay is optimal.
Working hours: Full time. Working hours can be flexible but 7-day cover required approx. 8-4 / 9-5-time frames.

• To have developed, through experience and postgraduate training, the specialist knowledge and clinical skills required for the post.
• The post holder will work in close co-operation with colleagues within all Care Groups to deliver a cohesive service that is user led ensuring a consistently high standard of patient care, optimal patient flow and safe timely discharge.
• To work closely with team co-ordinators and senior therapists to ensure a robust structure of training, supervision and appraisal is delivered to all members within the team.
• To work closely with team co-ordinators and senior therapists to ensure the delivery of an efficient operational service.
• To act as a source of Specialist advice and support within the Therapy Centre and across the Trust and the wider health economy.
• To have highly effective communication skills and demonstrate these on a continual basis with patients, carers, colleagues and the wider Multi-disciplinary Team (MDT).
• To undertake audit as a part of the role.
• To work flexibly as part of the cover arrangements within the Therapy Centre as required. The post holder will only be asked to work within their competencies and should be aware that notification of the need to move to a different clinical area may be communicated with little notice to ensure the safety of the service.
Working hours: 7 day working pattern - 8.30 to 16.30. Weekend working on rota basis. Will accept full time and part time positions.

The ward is led by a consultant and multidisciplinary team and require the support of therapists to support the ward team. Each ward has 20 beds and support is required across both sites but more urgently RSH Shrewsbury. The service requires one FTE Physio/OT/ and 2x Rehab Techs for RSH ward area, any interest for PRH can also be reviewed, working hours can be flexible but 7-day cover required approx. 8-4 / 9-5-time frames.

Unregistered rehabilitation/therapy assistant is required to provide ward based exercises and therapy under the direction of a registered therapist. The individual must be able to follow these plans independently and also identify low level equipment needs for patients to support their rehabilitation and plans to discharge home. Experience of ward-based therapy is essential, community based experience would also be desirable.

The post holder must be able to form key working partnerships across health and social care, working flexibly and creatively ensuring that the patient remains at the centre of the delivery of the services. They will also provide supervision and operational line management for clinicians working within the OPAT service. They will ensure that the patient’s pathway through the OPAT service is managed in a person centred, collaborative and proactive way. Skills/experience: Relevant professional healthcare degree or qualification (Nursing, Physiotherapy, Occupational Therapy, Paramedic). Registered with Health Care Professional Council or Nursing and Midwifery Council. Healthcare Leadership qualification or equivalent experience. Training in the Rehabilitation of the Older Adult/ Frailty/Long Term Conditions or equivalent experience. Working hours: Full time. Monday to Friday, 09.00 – 17.00.
