Job details:  Out of Hospital Care. Acute stroke. JD available on request: The Acute Stroke Occupational Therapy service provides specialist assessment and intervention for patients following a stroke on East Dean and Sovereign wards. Sometimes there are also outlying stroke patients on Berwick ward. The team work very closely with the stroke physiotherapists as well as working with other members of the MDT. They are split into two teams: triage and rehab. 
The triage team will complete initial assessments on patients admitted to the ward with stroke/suspected stroke. These assessments will focus on assessing a patient’s physical, cognitive and visual needs, as well as looking at their occupational performance and environment. Patients remain with the triage team if they require little to no therapy input prior to discharge. If a patient is deemed to require more intensive therapy rehab, they are transferred to the rehab team. 
The rehab team see patients who require more therapy rehab prior to discharge. These patients may discharge to home +/- additional support; the inpatient rehab unit (BIU) or the discharge to assess pathway. The rehab team are also responsible for providing proactive therapy groups.
The clinical work is largely in the acute setting at Eastbourne but may also require assessments in clients’ homes, covering an area which can include Bexhill, Lewes, Uckfield and Eastbourne.
Car driver essential. Preferable to have own car but pool car may be available upon request (dependent on demand and availability). Working hours: Shifts, 08.30 – 16.30. Monday to Sunday. Full time.
