Job details:  Highly specialist SaLT required to start asap for a minimum 3 months. Face to face role. Duties: Speech and language therapy in Luton specials schools including AAC and dysphagia. JD available on request: the post holder will provide highly specialist clinical input to the special schools in Bedfordshire. They will have day-to-day responsibility for coordinating a speech & language therapy service to special school children and independently manage the caseloads in these settings.  They will:

  • Use highly specialist clinical skills to manage a caseload of children with complex difficulties in speech, language, communication and/or eating and drinking (dysphagia) and AAC.
  • Identify, assess, and diagnose communication/swallowing impairments and determine appropriate intervention for effective treatment.
  • To assess children and provide written recommendations and ongoing support for implementation, for class teams.
  • Attend and provide reports to Annual Reviews/Multi Agency Meetings as appropriate.
  • Provide Communication Support and advice in relation to functional communication, to support teaching and learning.
  • Demonstrate specific speech and language therapy support programmes and the use of equipment to specific teachers or groups of staff.
  • Provide guidance and whole school training in relation to functional communication including receptive, expressive, and interactional skills. 
  • To contribute to relevant research, audit, and development work within the schools, relating to the Speech Language and Communication curriculum.
  • To attend relevant meetings as required within school or as part of wider SLT team.

Job details:  To start asap. Integrated Early Supported Discharge and Community Neuro Service. Some aspects of the role are done remotely but the main part of the role requires staff to see patients. Patient care is provided in the community and / or in one of our inpatient wards and most admin, meetings etc. are done remotely. Where possible, we are also encouraging telephone and attend anywhere appointments for patients too, but this is limited for the service due to the patient cohort supported. 
•To assess, plan and develop specialist physiotherapy programmes for patients under care of the Early Supported Discharge Service for Stroke (ESD). 
•This will involve working in a range of ward settings, outpatient departments and various community settings. This will include lone working within the community with access to Advanced Practitioners/band7 when required. 
• To provide a high standard of physiotherapy
• To perform advanced physiotherapeutic assessment of patients with neurological conditions including stroke and to deliver an individual treatment programme.
• To hold responsibility for own caseload and be responsible for a defined area of the service or a particular patient type, working unsupervised.
• To work as an autonomous practitioner within the multi-professional team.
• To provide support, regular formal training, clinical reasoning and peer review.
• To be accountable for assessing, interpreting, planning, implementing and evaluating treatment to patients within professional guidelines, with the support of Advanced Practitioners / Band 7.
• To identify patient needs, agree goals and provide appropriate therapy interventions relating to their physical, functional and psychological needs.
• Provide appropriate equipment and rehabilitation programmes, and make referrals on to other professionals and agencies as appropriate in order to meet the identified needs.
• To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
•To allocate work to non-registered staff.
•To provide neuro advice to professionals from other disciplines.
• Provide support and education to peers, new staff, non-registered staff and students.
• To supervise physiotherapist, non-registered staff, overseeing patient intervention and ensure that they deiver high quality patient care.
• To be actively involved in raining, teaching and supporting junior staff, non-registered staff to achieve competencies.
• To be accountable for a delegated caseload held by junior registered staff an non-registered staff in the team.
• To be involved in coordinating and providing 6-month reviews for stroke survivors.
• To build up and maintain key relationships with referring services and CNRS to facilitate appropriate referrals and an understanding of the roles within the ESD team.
• To take an active role in the clinical triage duty process within the ESD service to prioritise and ensure timely response for assessments.
• To be professionally and legally accountable for all aspects for your own and delegated work.
• To work within codes of practice and professional guidelines.
• To undertake specialist holistic assessment (including those with complex presentations and multi pathologies) making use of clinical reasoning skills.
• To use evidence-based practice, to ensure clinical care is effective and appropriate.
• To formulate and deliver an individual physiotherapy programme based on knowledge of evidence-based practice and treatment options.
• To evaluate patient progress, reassess and alter treatment programmes when required. 
• To understand and be acutely aware of significant medical observations during assessment and treatment of patients’ and report significant changes in patients’ conditions to the appropriate medical professional eg. named nurse, GP etc.
• To ensure smooth transfer of care which will include discharge from the acute or ESD services.
• To work within the Trust, CSP and HCPC guidelines, demonstrate a good working knowledge of nation and local standards of practice in line with legal and operational requirements and use own professional judgement to vary from established practices where required.
• To promote independence and wherever possible to avoid hospital admission and reduce inpatient stay to ensure those patients receive the appropriate care in the most appropriate setting.
• Organise and manage own time, delegating work appropriately.
• To conduct and contribute to risk assessments and health and safety assessments and including immediately reporting any changes/newly identified risks.
• To be responsible for the implementation of resulting recommendations from risk and health and safety assessments.
• To demonstrate the importance of gaining patient and carer consent in all interventions.
• To effectively communicate verbally with patients, carers and colleagues using tact and persuasive skills. This may involve using skills where patients have difficulties in communications eg. hearing loss, diminished sight, depression, aphasia, speech problems, cognitive impairment, behavioural problems and pain.
• Take an active part in the development of the service and in supporting the organisational aims and objectives of organisations models of care.
• To contribute to and work towards the service/organisational aims and objectives of the Integrated ESD and CNRS.
• To work effectively within the service and liaise appropriately with members of the MDT and other agencies by attending MDT meetings, case reviews.
• To ensure that accurate and timely written records are kept which comply with the Trust policy and HCPC guidance, reporting on any issues as appropriate.
• To provide specialist physiotherapy advice to all members of the MDT, service users and appropriate others of changes involving current care plans, patients progress and other relevant matters that pertain to the care of the patient.
• To support Advanced Practitioner/ Band 7 with day-to-day planning and delivery of a high quality service.
• To deputise for the Advanced Practitioner/ Band 7 for the management of patients.
• Travel independently around work base locality.
