Job details: Locum required for Buckinghamshire Perinatal Mental Health Team. To start asap.
Accept and triage all new referrals received by the team.
To communicate /liaise effectively with relevant referring partners and other agencies (Contact referrer to downgrade referrals (if required)/requesting more patient information).
Arrange and carry out initial assessments.
Sign-post patients to relevant services for support (if required)
To implement and /or participate in step-up care measures with patients/partners in conjunction with other agencies.
Review patients requiring short-term input from the team.
Responsible for responding to all duty calls Monday to Friday between 9am-5pm and putting in place appropriate actions to address identified risks.
To ensure that any identified immediate risks are appropriately responded to and a proportionate risk management strategy is actioned, with input from the multidisciplinary team members.
Ensure Patient Safety Board is regularly reviewed throughout the shift.
Identify/complete tasks to be undertaken that day and updated Patient Safety Board.
Discuss new referrals /assessments at Clinical Team Meeting every Wednesday.
Deal with urgent patient issues in the absence of the allocated Perinatal Practitioner.
Check out of hours report for any pending actions pertaining to Perinatal patients.
To accurately document all relevant information regarding patients in keeping with Trust guidelines. Etc.
Support student nurses/midwives
Skills/experience: Care Coordination, Assessment, Duty, Step-up-care, Perinatal Mental Health Community experience. Working hours: 37.5 hours per week. Monday to Friday, 9am-5pm with some working from home.
